What Is Pityriasis Rosea?
Pityriasis rosea is a common, self-resolving inflammatory skin rash believed to be triggered by reactivation of human herpesvirus 6 or 7. It typically resolves on its own within six to twelve weeks but can cause significant itching and cosmetic distress in the meantime.
The Herald Patch
Pityriasis rosea almost always begins with a single large, oval, salmon-colored patch — the “herald patch” — that appears one to two weeks before the main rash erupts, often on the trunk.
The Secondary Rash
A widespread eruption of smaller oval patches follows, typically arranged along the skin’s tension lines in a distinctive “Christmas tree” pattern across the back, chest, and abdomen.
Atypical Presentations
In some patients — particularly those with darker skin tones common in Hawaii’s diverse population — pityriasis rosea can present with inverse distribution, papular lesions, or significant hyperpigmentation rather than the classic pattern.
