Understanding Acne: Beyond Face Washing

Acne is a common dermatological concern, often misunderstood as merely a result of poor hygiene. However, its roots lie deeper, primarily in the interaction between hormones and oil glands.

Hormonal Influence: Acne formation is predominantly triggered by hormonal fluctuations, notably during adolescence and hormonal cycles in females. Genetics also play a role, influencing the sensitivity of oil glands to hormonal changes.

Comedones: The initial phase involves pores becoming clogged with skin cells and oil. ‘Blackheads’ (open comedones) form when exposed to air, while ‘closed comedones’ develop deeper within pores without darkening.

Inflammation and Bacterial Infection: Inflammation in acne is primarily fueled by P. acnes bacteria, thriving in low-oxygen environments within clogged pores. This bacterial overgrowth leads to the red, swollen appearance of pimples.

Treatment Approaches: Effective acne treatment revolves around unclogging pores, combating bacterial overgrowth, and regulating hormonal influences.

  • Unclogging Pores: Retinoids and keratolytics such as retinol, tretinoin, salicylic acid, and glycolic acid work to prevent clumping of skin cells and oil within pores.
  • Killing Bacteria: Topical benzoyl peroxide and systemic antibiotics target P. acnes bacteria, with prescription antibiotics considered for severe cases.
  • Regulating Hormones: Oral contraceptives and spironolactone are options primarily for female patients to control hormonal influences on oil gland activity.
  • Systemic Treatment: Isotretinoin, known as Accutane, is a potent medication reserved for severe, treatment-resistant acne cases, offering long-term reduction in acne but requiring close monitoring due to potential side effects.
