What Are Actinic Keratoses?
Actinic keratoses (AKs) are rough, dry, scaly patches of skin caused by years of cumulative UV exposure. They are considered pre-cancerous lesions — not skin cancer themselves, but a sign that the skin’s DNA has been damaged enough that, without treatment, a meaningful percentage will eventually develop into squamous cell carcinoma. They most commonly appear on the face, scalp, ears, neck, forearms, and the backs of the hands.
Cumulative UV Damage
AKs are a direct result of repeated, long-term sun exposure. In Hawaii, where UV index levels stay high year-round and outdoor living is a daily habit, that cumulative damage builds faster than in most parts of the country — making AKs especially common among our patients.
Risk Factors
People with lighter skin tones are at significantly higher risk for AKs. Risk increases steadily with age as DNA repair mechanisms slow down and decades of sun exposure accumulate — though AKs can appear in younger patients with a heavy sun history, as is common in Hawaii.
Immunosuppression & Other Risk Factors
A weakened immune system — from illness, organ transplant, or certain medications — significantly raises AK risk, as does a personal or family history of skin cancer. Outdoor occupations, a history of tanning bed use, and living at high-UV latitudes like the Hawaiian Islands all compound that risk further.
