Actinic Keratoses
Years of Hawaii’s intense sun have a way of leaving their mark — and one of the most common signs is actinic keratoses. These rough, scaly patches may seem minor, but they are the earliest warning signal that sun-damaged skin cells are moving in the wrong direction. Catching and treating them now means a simple in-office procedure instead of something far more involved later. At Hawaii Premier Dermatology, we help Honolulu patients stay ahead of sun damage before it becomes something harder to reverse.

What Are Actinic Keratoses?
Actinic keratoses (AKs) are rough, dry, scaly patches of skin caused by years of cumulative UV exposure. They are considered pre-cancerous lesions — not skin cancer themselves, but a sign that the skin’s DNA has been damaged enough that, without treatment, a meaningful percentage will eventually develop into squamous cell carcinoma. They most commonly appear on the face, scalp, ears, neck, forearms, and the backs of the hands.
Cumulative UV Damage
AKs are a direct result of repeated, long-term sun exposure. In Hawaii, where UV index levels stay high year-round and outdoor living is a daily habit, that cumulative damage builds faster than in most parts of the country — making AKs especially common among our patients.
Risk Factors
People with lighter skin tones are at significantly higher risk for AKs. Risk increases steadily with age as DNA repair mechanisms slow down and decades of sun exposure accumulate — though AKs can appear in younger patients with a heavy sun history, as is common in Hawaii.
Immunosuppression & Other Risk Factors
A weakened immune system — from illness, organ transplant, or certain medications — significantly raises AK risk, as does a personal or family history of skin cancer. Outdoor occupations, a history of tanning bed use, and living at high-UV latitudes like the Hawaiian Islands all compound that risk further.
The staff was pleasant, the wait was short, and the diagnosis and treatment was very straightforward. Dr. Musaev has excellent knowledge and explained things well. The procedure was quick and painless. Clinic location and parking was easy. A great community resource!

Great experience at Hawaii Premier Dermatology! This was my first derm appointment and I am very happy I chose to come here. Justin was very knowledgeable and helpful and I even got something else checked that I didn’t originally plan on getting checked. I’ll be coming here from now on and I highly recommend!

Justin Maio is #AMAZING. Such a wonderful person with a great personality, and he always puts you at ease–even when it can be scary getting biopsies! I would travel 50 miles anywhere on this island just to see him–and I already go 22! Best office with the nicest and most professional staff. I truly cannot say enough positive things–No joke!

Had a worry about a skin cancer spot and was relieved when Dr. Tagai had a spot for me that week. He had excellent bedside manner and was compassionate when I was a bit of a wimp (okay, a big wimp) with the needle. Highly recommend him.

After two years of wondering and self treating and many healthcare visits to the cheap clinics and constant AI asks, I paid out of pocket less than $400 for 2 visits to find out I have a rare skin condition called Actinic Dermatitis. Thank you Justin, now I know and now I can sleep well at night and understand what is NOT causing this. I really appreciate your expertise!
Tim Swartz

Precision Diagnosis
Because actinic keratoses can sometimes resemble other skin conditions — and because catching the difference between a pre-cancerous lesion and an early squamous cell carcinoma matters — accurate diagnosis is critical before any treatment begins.
- Full-Body Skin Examination:
We recommend a thorough head-to-toe exam to all patients with diagnosis of actinic keratosis
- Dermoscopy:
High-magnification imaging lets us examine the structure of each lesion closely, helping distinguish a standard AK from one showing signs of early progression.
- Skin Biopsy:
When a lesion looks atypical or has features that raise concern, an in-office biopsy provides a definitive answer and rules out squamous cell carcinoma.
Knowing exactly what you’re dealing with is what allows us to match the right treatment to the right lesion — and to monitor your skin effectively over time.
Advanced Treatment Pathways
Treating actinic keratoses now is one of the most effective things you can do to protect your long-term skin health — and our Honolulu team is here to make that process as straightforward as possible.
- Cryotherapy (Liquid Nitrogen): The most common treatment for individual AKs. We apply liquid nitrogen directly to the lesion, freezing and destroying the abnormal cells quickly and precisely.
- Photodynamic Therapy (PDT): A highly effective option for patients with multiple or widespread AKs. A photosensitizing agent is applied to the skin and activated by a specific light source, selectively destroying pre-cancerous cells across a broad treatment area.
- TCA (trichloroacetic acid) is a medical-grade chemical peel applied in-office to treat actinic keratoses, removing sun-damaged cells from the skin's surface and encouraging healthy new skin to take their place.
Field therapy uses a prescription cream applied at home over several days to weeks to treat both visible actinic keratoses and hidden sun-damaged cells across an entire area of skin, helping reduce the risk of new lesions forming.
- Scheduled Skin Checks: Because AKs frequently recur in sun-damaged skin, we establish a regular monitoring schedule to catch new lesions early.
- Reef-Safe Sun Protection: Daily broad-spectrum mineral sunscreen is non-negotiable for AK patients in Hawaii — we help you find options that comply with the state’s reef-safe regulations.
- Sun Damage Counseling: We provide practical, Hawaii-specific guidance on reducing ongoing UV exposure without giving up the outdoor life you love.
